• If you have not yet turned in a signed copy of the Mandatory Parent/Guardian Signature page to acknowledge receipt of the listed information, please print a copy, and return to the school office at 470 Searls Avenue, Nevada City, CA 95959.

• An updated emergency information card and the mandatory signature page must be signed and returned annually to the registrar’s office at 470 Searls Avenue, Nevada City, CA 95959.
